With due respect,it is humbly submitted that my full name in the matriculation
certificate and professional course of Art & Craft is Ramesh Kumar Negi,
whereas the same has been written as “Ramesh Kumar “ in my appointment
orders and other official records like Service book etc. Surname was also not
mentioned in the category certificate submitted by me which has now been got
rectified and copy of the same is enclosed herewith for information and
necessary action.

In view of above explained position, your goodself is requested

very kindly to add the surname in my all official records and also issue
corrigendum in this regard in order to avoid any complication in this behalf.
