Caution – If the vehicle is found to be in need of repair or in any way unsafe, or contributes to an unsafe condition,
the matter shall be reported immediately to the designated authority, and the vehicle shall not be operated until it
has been restored to a safe operating condition. If during operation, the truck becomes unsafe in any way, the
matter shall be reported immediately to the designated authority, and the truck shall not be operated until it has
been restored to safe operating condition. Do not make repairs or adjustments unless specifically authorized to do
so.

Damage – Bent, Dented or broken Electrical Safety Devices-
parts Horn, Audible signals, Entry
bar, flashing lights, Indicator
lights, etc. Work Properly
Leaks – Fuel, Oil, Radiator, Steering – No binding, No
Hydraulics excessive play
Tires & Wheels – Drives Wheels, Brakes & Travel Controls –
Steer Wheels, Load wheel, Casters, Check Braking while Traveling
Etc. slowly at first. If ok, then at
higher speed. Acceleration
smooth, not jerky.
Chains, Cables & Hoses – In place Parking Brake – Seat, hand or
foot
Battery – Battery Cable and Hydraulic Control – Test all
connector condition. Cable routing hydraulic functions for smooth
(not outside of truck). Battery and proper operations and no
retainer securely in place. usual noise
Guards – All Guards in place and Attachments – Function
condition. Overhead, backrest, etc. properly, No unusual noise,
securely in place and
removable attachments
Mechanical Safety Devices – Limit Switches – Limit
Operator harness or seat belt, switches, Tilt switches, Travel
name/ data plate, warning labels, limit, Safety reverse (walkies),
etc. In Good condition. etc.
Operator Compartment/ Controls Power Disconnect – Cut- off
– All controls, steer tiller, pedals, all electrical power
etc. in good condition. Seat locks in
position
Power Up – Turn on key. All
indicator lights and alarms are
working
.
