Repuhtic ofthe Philippines Sapreme Genet Office of the Geart Aawiuistrater Baits OCA CIRCULAR NO.170-2020 To : ALL JUDGES AND COURT PERSONNEL OF LOWER COURTS Subject : ONLINE FILING AND SUBMISSION OF DOCUMENTS FOR APPOINTMENT IN THE LOWER COURTS Considering the public health crisis brought about by the COVID- 19, the online submission of the following documents for appointment in the Lor He wer Courts in Portable Document Format (PDF) shall be allowed: List of Indorsed applicants; 2. Certification under oath by the Presiding Judge/Executive Judge Nog that the list submitted contains the name/s of all the applicant/s who applied for the position pursuant to OCA Circular No. 74- 2010; .. Certification under oath by the Clerk of Court (COC in the Office of the Clerk of Court [OCC] for vacancies in the OCC or COC in the branch for vacancies in the branch) that the position to be filled up has been posted in three (3) conspicuous places in the court’s premises and other public places for a period of at least ten (10) calendar days pursuant to R.A. No. 7041, indicating the date when the notice was posted and the public places where it was posted pursuant to OCA Circular No. 74-2010, as amended by OCA Circular No. 44-2013; (PLEASE USE THE PRESCRIBED FORMAT) . Selection and Promotion Board-LC Forms: Recommendation Form (Form 001), Interview Results Form (Form 002), and Examination Results (Form 003) prescribed under OCA Circular No. 128-2016; . Favorable recommendation/Indorsement of Presiding Judge; . Favorable recommendation/ Indorsement of the Executive Judge; - Comment of the Clerk of Court on the application(if vacancy is in the Office of the Clerk of Court); .. Five (6) copies of properly, completely and recently accomplished and notarized Personal Data Sheet (CSC Form 212, Revised 2017) with latest 3.5 cm x 4.5 cm (passport size) pictures, including the Work Experience Sheet (five (5) copies); (PLEASE USE THE PRESCRIBED FORMAT) . Authenticated copy of Civil Service Eligibility from the CSC Regional Office and official receipt of payment for authentication (3 copies) or authenticated copies of Board Rating and Professional Regulation Commission (PRC) License from the PRC (B copies) (for positions requiring civil service eligibility); 10. Bar Rating and Certificate of Membership (two (2) copies each) in the Philippine Bar (for Clerks of Court requiring lawyer Positions); 11. Transcript of School Records duly authenticated by the School Registrar or other authorized school official; (FOR TRANSFEREE AND ORIGINAL/REEMPLOYMENT APPOINTMENT) 12,Sworn Affidavit executed by the applicant/recommendee that he/she has no relative(s) within the third civil degree of consanguinity or affinity who is an incumbent employee in the subject branch/ office; (FOR TRANSFEREE AND ORIGINAL/REEMPLOYMENT APPOINTMENT) 13,Properly accomplished and signed Position Description Form (DBM-CSC Form No. 1, Revised Version No. 1, s. 2017), four (4) copies; 14.Birth Certificate issued by the Philippine Statistics Authority (PSA), 2 copies; (FOR TRANSFEREE AND ORIGINAL/REEMPLOYMENT APPOINTMENT) 15.Certificate of Marriage issued by the National Statistics Office (NSO), 2 copies; (for married women); (FOR TRANSFEREE AND ORIGINAL/REEMPLOYMENT APPOINTMENT) 16.Sworn Affidavit executed by the applicant stating his/her willingness to accept and be appointed under a temporary appointment if he/she does not meet the experience and training requirements (for positions requiring work experience and/or training); 17.Certified photocopy of proof of completion of relevant training/seminar in a training institution accredited by the Civil Service Commission, 2 copies (if applicable); 18. Proof of completion of Secretarial/Stenography course or units in Stenography for Stenographer positions; 19.Report on the result of the competency examination, together with a copy of the shorthand notes and transcription thereof, pursuant to OCA Circular No. 124-2004 (for stenographer positions only). Please see attached copy of circular for particulars; 20.Certification issued by the Clerk of Court, duly certified by the Presiding Judge/Executive Judge as to the applicant's knowledge and proficiency in the use and operation of computer, for Data Entry Machine Operator II position; 21.Latest Medical Certificate (CSC Form No. 211) with attached original copy of laboratory results; if in case the same cannot be submitted yet, the applicant for the meantime can submit affidavit that he/she is fit to work subject to the submission of the said medical certificate; (FOR ORIGINAL/REEMPLOYMENT APPOINTMENT) 22.Latest NBI Clearance; (FOR ORIGINAL/REEMPLOYMENT APPOINTMENT) 23. Latest Performance rating (IPCR) Form (2 rating periods); (FOR TRANSFEREE AND PROMOTION APPOINTMENT) 24, Clearance from present office indicating no money and property accountabilities; 25.Certification from present office indicating no pending administrative and criminal case; 26.Conformity on the application to transfer from the Presiding Judge/Executive Judge of the present station/office for lower court personnel/ Authority to Transfer; 27.Certification from present office of no pending transcription of stenographic notes (for court stenographers); 28.In cases of demotion in salary or in rank, written manifestation on his/her willingness to accept the salary rate of the position applied for; 29. Latest Authenticated copy of service record; 30. Certified photocopy of last approved appointment; and 31.Certification of employment from present and previous office, indicating therein the period of employment and statement of duties and responsibilities.
