Roleplay about Human Physiology and Anatomy of

Integument (skins)
 
At one of health center in Denpasar the arrival of a patient named Alika who was ten years old
with her mother named Melati. On Wednesday, March 15th 2018 at 08.00 WITA with complaints
of itching and feeling hot on the skin of her right hand.
Nurse : Good morning, ma'am, can I help you?
Mother : Morning sus, from yesterday my child feels itching in her right hand, after the itching
that will feel hot but there is no redness on her hand.
Nurse : Girl ..., introduce myself, my name Novi, can you tell me, what is your name ?
Patient : My name is Alika sus (scratching her hand)
Nurse : How old are you?
Patient : Ten years old sus
Nurse : Can I check your right hand ?
Patient : Ofcourse sus, please (the patient immediately gives her hand)
Nurse : After I had checked her right hand, from the symptoms of the child's hand is a symptom
of eczema or dermatitis
Now, I will explain a little about what eczema or dermatitis is, which the skin looks inflamed and
irritated. The main symptom of eczema is itching but there is no sign of redness on the skin. The
affected area will feel very dry, thickened, or scabbed. Usually eczema will affect the skin
pigment so that the eczema area will appear brighter or darker.
Mother : What makes my child feel itching like this?
Nurse : It’s cause eczema related to excessive immune defense (immune) activity. Causes the
body to overreact to bacteria or irritants that are harmless to the skin. So eczema is found in
families with a history of allergic disease. Eczema suffered by you may be from dust directly
exposed to the bacteria.
Now, I will explain a little antomically and physiology of the skin. Where the skin plays an
important role preventing the entry of bacteria , chemicals and the sun (ultraviolet radiation). The
skin is divided into three layers, namely, the first epidermis is the outermost or top part of the
skin derived from ectoderm consisting of several layers (multilayer), functioning as an essential
nutrient exchange place. Secondly there is the dermis, which is 95% of the dermis forming the
skin thickness is considered as "True Skin". The dermis contains special cells that help regulate
temperature, fight infection, supply blood, and nutrients to the skin. The third is the deepest part
of the skin hypodermis. In this layer there are edge nerve endings, blood vessels and lymph.
Hypodermis contains fat tissue, lymphatic blood vessels. So the eczema that is suffered by a
mother is in the outermost layer of the skin, the epidermis, which is directly exposed to bacteria,
the occurrence of itching and irritation of the skin .
Mother : How do you get rid of itching on my child's hand or is there a way to treat it?
Nurse : Please calm there must be a way of treatment or relieving itching in the hands of your
child. When the skin feels very dry and itchy, moisturizing lotions and creams are recommended
to make the skin more moist to prevent infection. Cold compresses can also reduce the itching.
Ointments or creams containing teroid carticos such as hydrocortisone are given to protect the
inflammatory process or inflammation. If the area of eczema has been infected then antibiotics
can be given to kill the bacteria that cause infection.
Mother : How to prevent it sus?
Nurse : How to prevent it very easily, can be avoided by:
        Avoid sudden changes in temperature and humidity
        Avoid sweating too much or overheating
        Avoid soap with ingredients that are too hard, detergent and other solutions
        Be careful in choosing foods that cause allergies and environmental factors such as dust
and animal dander
Mother : Thank you for her suggestion sus, I'll do it at home for the sake of healing and health of
the family especially my daughter
Can this disease be contagious?
Nurse : Yes, you are welcome . Eczema is not contagious and does not spread to others.
Come on girl, I will give the ointment to your hand to get rid of the itching
Patient : Does it hurt me?
Nurse: The pain is only a little, this is for the healing of you to eliminate the itching
Patient : Okay sus, (the patient immediately gives his hand)
Nurses also applied ointment to relieve the patient's hand in order to get a l-itch and prevent
infection. After a few minutes, the patient leaves home
Nurse : I have finished giving the ointment to your daughter hand. I suggest that when given
ointment, don't scratch it or scratch it and hold it a little. This is for the ointment , please use it in
the night.
Patient : Okay sus (smiling)
Mother : Yes sus (while taking the medicine)
Nurse : I suggest that you must supervise your children from environmental factors, especially
dust because the dust is exposed directly by bacteria that will enter the skin of his hand which
will cause infection and also in food, nutritional intake should be enough if the child has food
allergies heart in choosing food.
How to use the ointment, you can apply it twice a day during the day and night.
