1. EXECUTIVE SUMMARY
A Risk Assessment was held in purpose of re-identification the risk possibly occur during work road
crossing working at height in EPC1 Banyu Urip Project. The Risk Assessment covered the EMDC
working at height expectation. The actions were determined to ensure coverage all the expectation
that able to minimize the risks as low as reasonably practicable.
The meeting was held on SundayThursday, 010 4 FebruaryJune 2013 at 1316:330 to 168.00 hrs.
The objectives of this Risk Assessment are to:
 Identify hazard and project risk issues associated with the project common activity in working at
height. work road crossing.
 Identify mitigations (if any) to achieve acceptable risk.
Propose the mitigation plan to be used for improving the safety working methods for any activity
deal with work road crossingworking at height.

As a result some actions need to be taken to allow risk as low as reasonable practicable

1.1 Work Process Summary


The risk assessment was initiated by MCL in response to the input given by Exxon CRR team,
where work road crossing working at height is one of the big concerns right now.
The work process was started with brain storming and continued with determining the critical
scenarios that lead to the incident. All participant were actively contribute and many ideas were
captured in this process
The team then developed the risk scenario worksheets. The risk scenario worksheet identify
potential scenarios and document the potential scenario causes, consequences, assumed
safeguards and proposed action items. The risk scenario worksheet also document the assessed
risk in terms of health and safety, environment, public disruption, direct financial impact and
business impact; both before and after the proposed action items. The risk scenario worksheets
are presented in section 3 of this report.
The risk ranking is based on the information available during the risk assessment and assumes
that the identified safeguards are in place and effective.

1.2 Result of Summary


A total twenty-fivethree (153) hazards and concerns were identified during the risk assessment.
All hazards were consolidated into three two (32) risk scenarios.
The assessed risk level before mitigation action items was
 Two(2) in Zone 1 risk
 ThreeTwo (32) in Zone 2 risk
 OneTwo (2) (1) in Zone 3 risk
The assessed risk level after proposed mitigations are put in place are
 Two (2) in Zone 2 risk
 Two (2) in Zone 3 risk
 Two (2) in Zone 2 risk

The role of the Risk Assessment team ends after submission of the report. The PIC will be
assigned to resolve the assessment finding.
It is important to recognized that although the proposed action items may provide adequate
safeguards for the identified hazards. Tthe parties responsible for developing the action plans
should not restrict their planning to the actions or methods presented if additional or other
measures are deemed appropriate.
