On date 11th Jan 2021 vehicle came workshop for brake complaints. During repair it has been found that
shock absorber, top strut mount damaged. Also brake pad was totally worn out which damaged the
wheel disc. These all damaged explaining careless and rough driving of vehicle which is totally
unexpectable.

Therefore, the following disciplinary action will be taken against you:
