*I have read the Shuttle Bus Guidelines and do understand its importance.

I promise to
advice and instruct my child to follow these guidelines.
*When an emergency occurs and the school is unable to contact either a parent or an
emergency contact, it is understood that the school has the parent's permission to
arrange emergency medical care for the child and administer the indicated medical
response or medical intervention.
*A regular part of The Child program is to include class field trips which involve taking the
children off the school premises during the school hours. These purposeful trips are
specifically designed to enhance your child’s education and social development. These
are also crafted to support specific educational needs
essential in their learning.
*All field trips are chaperoned by teachers at all times. You will be informed of the class
field trips and will be reminded about these activities several days earlier.
This Permission Form is to provide the school with your agreement on your child’s full
participation in all class field trip offered throughout the school year.
*We, therefore, ask for your signature and to return this form, so your child can fully
participate in these valuable and exciting activities as part of being a student at THE
CHILD.
*I do understand the statements above and give permission to my son/daughter to
attend the school-planned learning activities. I understand that these field trips will be
under the supervision of the THE CHILD’s teaching staff.
*In order to fully meet the needs of your child, it is important that we receive necessary
and essential information from your child.
* List any foods/drinks that your child should not have:
*Does your child have any health problems?
*Do you have any specific concern about your child’s physical development?
*Does your child play well in groups?
*Has your child gone to any other kindergarten, preschool, or day care before?
If so, please describe your child’s previous experiences.
