I was excited to see the advertisement for LPN within Northern Lakes Hospital, I am currently
completing my last semester of the LPN program and I am excited to start my career. I excel in
stressful environments and prioritizing tasks based on the level of importance. I am an excellent
multitasker, demonstrated as I have spent the last year homeschooling my three children and
meeting and surpassing my targets as a student nurse. I pull from my experiences in life to guide me
in my decision process. I am an activist at heart and strongly believe in advocating for my clients,
within my scope of practice, to provide them with holistic care. I have had an eclectic work
experience that includes years of volunteer experience within North America and sub-Saharan
Africa, ranging from river restoration in Northern BC to canvassing throughout the USA to
HIV/AIDS awareness and gender barrier workshops in Children’s Town, Zambia.

These experiences have shaped who I am as a person and naturally flow into my nursing practice. I
look forward to caring for others and utilizing what I have learned and what I will learn in the years

Professional Profile

Personal Philosophy of Practical Nursing

Oxford dictionary states that Philosophy is "a particular set or system of beliefs resulting

from the search for knowledge about life and the universe." I believe that the search for

knowledge is a life long journey that will mirror my nursing career. My philosophies will not

remain static throughout my nursing career. They will change as I gain more experience, but as I

set out on this new journey, I can clearly articulate which values I embrace at this time. I want to

be an advocate for my clients, to provide clients with the knowledge they need to be involved in

their care and to make informed decisions regarding their health. I want to do this by providing a

holistic environment for my clients.

My philosophy originates from beliefs and values that were inculcated in me throughout

my life experiences as a child and now as an adult. These experiences have provided me to

witness how important advocacy is, for myself and for the people around me. I am excited to be

a support for my clients through compassionate care, the client's wellbeing and treat them with

the same love and care I would expect for my loved ones.

We live in a cultural mosaic where not everyone communicates in the same language,

values and beliefs vary significantly from person to person. As a nurse, I will appreciate the fact

that people come from many walks of life. I will acknowledge this by being aware of services

and resources that may be available to my clients based on their individual needs. I believe a

large part of educating clients begins by learning more about each client and who they are as a

human. I want to provide quality care individualized to each patient.

Potter and Perry (2018) state, "Although the mechanisms are not yet fully understood,

body, mind, and spirit are interconnected; when one facet of a person faces a challenge or receive

support, the others are affected...Research such as this suggests that spiritual and religious

practices may do more than just provide a comforting effect. Prayer, meditation, contemplation,

sacraments, rituals, and other practices that support spiritual and religious expression may indeed

reduce stress and support health and healing." I genuinely believe in the phenomena of how

holism affects our whole being.

I look forward to witnessing and participating in providing integrated care. Every day I

will work towards providing an environment of understanding of the client's values and choices.

I look forward to utilizing the skills and knowledge I have learned at Northern Lakes College, as

well as the insight I bring from my personal experiences. I have outlined my philosophy of

nursing care and embrace becoming a more practiced, competent nurse as my career progresses.

while I was seeking childcare for my then one year old. I could not have been more happy and grateful

that she was placed in our path. Julie cared for my child on and off for three years and I feel absolutely

blessed for that time. She was very patient and loving towards my child who was diagnosed with ADHD

and a speech delay. Julie greatly excelled with communicating with her. She would always get down to

her level, treat her with kindness and compassion and make her feel heard. Leaving my daughter in her

care was truly special because she was treated as if she was one of her own children. Writing this letter

is bittersweet for us as we will miss her terribly as our Day home provider but we wish her nothing but

I first met Julie as part of Katimavik, Canada’s national youth volunteering program. Julie was

communities across the country. Their purpose was to develop leadership skills, learn about different

communities, and contribute to various non-profit organizations within the community. I was the

Project Leader during her time in William’s Lake, British-Columbia. It was a pleasure and a privilege

to work with Julie during that time.

Julie was a natural leader within the group. Other participants looked to her for support,

guidance, and decision-making. She taught many of the group members skills in patience, cooking,

and communication, in particular. Generally, she is thoughtful and caring and pays attention to the

Learning Goals and Planning

Learning Goal/s: I will feel more confident in my knowledge of the disease process and how the

body interacts with medications, including the signs and symptoms to assist in preparation for

year two of my studies. This is a lifelong expectation of me in my career, but I will dedicate this

summer break to refreshing and solidify the core medications I expect to come across regularly.

Learning Plan: I will set a regular schedule during the summer to review one medication daily, I

will utilize an alarm to remind me to do so. I will utilize my pharmacology textbook, my drug

cards, the CLPNA website as well as YouTube videos to increase my depth of knowledge.

Though this is a lifelong competency, by the end of this break I will have a better understanding

of medication and the bodies response to them.

Competency application: A-11-3, Identify classifications, generic, and trade names of drugs and

medications and appropriate pharmalogical resources.

Reflection

What I learned: During this clinical placement it was helpful because it worked towards

increasing my knowledge of drugs. I was able to become more familiar with the common

medications utilized in long term care. I did so by utilizing my drug cards and discussing with

my classmates furthering my familiarity with the medications. I don’t think this goal will ever be

truly met because I know that I will be learning and adapting to medications/changes for the

length of my career.

Proficiency rating: I believe my competency is in between fair and good, I think this because I

am able to apply some of my knowledge in my nursing practice but not all.

Why it is meaningful/significant: As a nurse I will give medications every single day of my

career. I need to have a strong understanding of what I am giving to my clients, it is vital for the

wellbeing of my clients and for my nursing practice.

How I will apply this learning in nursing practice: I will be able to feel confident in providing my

clients with the medications they need, from something as simple as management to more

complex issues.

As an ongoing goal I don’t feel like I will be able to prove evidence of knowledge in my

portfolio, however I believe it will be something that will be evident during my nursing practice.

Introduction

As a nurse there is an opportunity, a responsibility to teach our patients to help better their

circumstances. I look forward to providing a patient-centered approach to patient education,

where the client is a part of the teaching process, working together to formulate a plan to

effectively solve their issues to best fit each situation. Potter and Perry states “nurses must use

both social learning theory and the nursing process…The L.E.A.R.N.S model…incorporates

both…

George Smarts Teaching Plan for Constipation

Assessment

George Smart is a 29-year-old male that came to address issues with constipation. He is

an immigrant to Canada as of 2 years ago and since arriving (with the shift in diet) he has been

struggling to have regular bowel movements. He is fluent in English. He works as a long-haul

truck driver with prolonged periods of sitting. He does not pack a healthy, nutritious lunch,

instead he finds himself stopping to eat high fat fast food wherever he can. Due to his diet he

recognizes he is not getting enough water during his work week. George presented as a visual

learner who enjoyed reading information as well as watching videos and interacting. His wife

joined him for his appointment and conveyed she would be an active participant in helping

George. George identified himself on the Bristol stool chart as a 1-2 every 2-3 days. He has

experienced hemorrhoids on more than one occasion after a severe bout of constipation.

Nursing Diagnosis

Constipation RT sedentary lifestyle, inadequate water intake, high fat diet as evidenced

by hard stool, straining and hemorrhoids.

Planning and Implementation

George has agreed to work on the following goals and recognized the importance of

making positive changes to help relieve him of constipation. Potter and Perry states “One of the

most important habits nurses can encourage patients to establish is a daily bowel routine that

includes adequate fluid and food intake as well as regular exercise.” (2019, p.1229) George has

agreed to drink 2500mL of water daily, he will meal plan and increase the fiber content of his

diet and be able to recognize foods high in fiber, he has agreed to increase his daily exercise.

According to Potter and Perry “Daily fluid intake should be between 1500 and 2000mL per day

of noncaffeinated beverages. Dietary fiber should be from 25 to 30 g per day…Even minimal

activity (such as leg lifts) increases peristalsis.” (2019, p.1227-1228)

Goals:

• George will start moving more regularly at work during his long wait times

utilizing a 7-minute workout application and the built-in pedometer on his

iPhone.

• George will increase water take by keeping a portable water bottle with him plus

keeping a larger jug in his truck where he can refill his bottle at work when he

doesn’t have access to water.

• George will learn healthy easy meals to pack to work with the assistance of his

wife and aim to eat a more traditional diet.

Outcome/SMART goal:

George agreed that within two weeks of starting his teaching plan he will establish

normal defecation, will be able to voice relief from constipation and be able to identify himself

on the Bristol stool chart between 4-5.

Individuals involved/required:

Georges wife has committed to actively participate in helping George be more

active by walking with him three times a week and assisting in packing balanced,

nutritious, fiber rich meals for when he is at work and at home.

Evaluation

Two weeks follow up, George was happy to say his constipation was gone and on the

Bristol stool chart he was between 4-5, daily.

Cognitive domain learning: He clearly articulated during questioning how he had learned

valuable new skills on eating nutritiously, having a more active lifestyle and drinking adequate

amounts of water. George can correctly identify high fiber foods. He plans on maintaining his

current plan.

Affective domain learning: George was relieved both physically and mentally. As a side effect to

the teaching plan he expressed feeling closer to his wife after spending the last two weeks

cooking and walking together.

Reflection

I feel one of the greatest strengths I have gained while working with George Smart was learning

how common constipation is. As a nurse I know I will come across it many different times

during my career, this teaching plan has given me a broader scope of natural interventions before

resorting to a pharmacological approach. The weakness is that this plan will not work for

everyone, there are many variables such as age, medications, diet restrictions, mobility deficits

and biological factors can all play a huge role in regular bowel health and wellness.

Professional goals: My goals include but are not limited to, advancing my education further by

becoming a lactation consultant. As one of my teachers said ‘Embrace learning, say yes to every

opportunity!’ I plan on doing just that! I would like to do some international work as a LPN, and

I would like to experience remote nursing.


Leadership and Mentoring: While performing clinical at one of the care facilities in Grande

Prairie, I was comfortable taking the lead in client care. It was easy for me to start and keep

conversations going and it created a relaxing, trusting environment for myself, my classmates

and most importantly my clients. We were also given many opportunities to critically think our

way through situations, with the guidance of our instructors. I found these moments invaluable

for my knowledge development and thought processes to find the correct solution.
CPRNE Study Plan: I will utilize the CLPNA as a component of my studying, as well as

Mosby's Comprehensive Review for the Canadian PN Exam. I will review my notes and course

work and study with classmates who are also preparing to pass the licensing exam. I plan on

writing and passing the CPRNE exam in September 2021.


Continuing Competency:

C-5-5 Demonstrate knowledge and ability to maintain confidentiality:

• adhere to legislation and regulation, professional standards and employer policies

• identify risks and report breach of confidentiality

• manage patient information appropriately

o administration of fluids and medications

I will utilize online research as well as facility policies and seek experience to shadow, and

manage hypodermoclysis infusion.
