    THIS IS TO CERTIFY that SANTIAGO ELEMENTARY SCHOOL has been converted to
    SANTIAGO INTEGRATED SCHOOL as per Government Permit (PHS) No. S-002, s. 2020 given last
    February 14, 2020 at City of San Fernando, Pampanga, Philippines.
    The said permit was received by the Schools Division of Zambales last February 22, 2019
    and was implemented last October 2020, the beginning of School Year 2020-2021 for the New
    Normal Education.
    This is to certify further that the School ID of this Educational Institution was changed from
    106812 to 501830 by virtue of the same government permit.
    This certification is issued upon the request of COMELEC.
    Given this 17th day of November, 2020 at Santiago Integrated School, Purok 3, Santiago,
