IAP - Incident Action Plan

BLUF:
Mahidol University International Demonstration School(MUIDS) had faced the chemical spill
incident occurring under the tent next to the football field and gymnasium. This incident action
plan is required to be written due to the accident and our objective is to rescue the victims,
remove the hazardous material spill, and prevent the environment surrounding. On the day, It’s
a warm sunny day which is about 37 degrees Celsius with a few clouds and is about half of
humidity. As we investigate the locale, there are five chemical containers, a random wooden
palette, and two victims suffering from the effect of the chemicals. Whereabouts the incidence
happens there is water drain down along the football field and parking lots. So we assume that
some of the chemicals might go down the drains. During the investigation, we found out that out
of five barrels there is only one without the chemical label. The four labels are polyurethane,
toluene, tetrachloroethylene, and sodium borohydride. From the photo that has been taken from
the scene, we can see that the only barrel that didn’t spill out is sodium borohydride, which
reacts with water and is flammable gas. It also can re-ignite and is soluble in water.
Furthermore, polyurethane is flammable, carcinogen, irritant, and resistant to water and oil. It
can cause asthma and breathing problems, and also can lead to headache and sore throat.
Toluene, very flammable, carcinogen, irritant, heavier than air, and will float on water. The last
chemical is tetrachloroethylene, which can form an explosive mixture with air. It’s toxic if
swallowed or absorbed through the skin. So, to achieve our objectives we need to rescue the
people first by evacuating the victims to the safety area with the fresh air and running their eyes
with water for at least 20 mins, removing and isolating their contaminated clothing and shoes
and then washing their skin with soap and water. This will clear the chemical out of their body at
the very first stage before going to the hospital. Calling 911 and ambulance to notify them of any
injuries they got from the chemical spill and send them to the nearest hospital which is Golden
Jubilee Medical Center, Faculty of Medicine Siriraj Hospital, Mahidol University. Also, warn
people in the school especially the gymnasium to be aware of the effects of the chemical using
the school announcement. Next is to remove harmful materials. Appropriate protective clothing
or suit level B are required before entering the scene. Firstly, use wood plug kits to prevent
chemicals from leaking out or go down the drain and control the spread of the liquid by using
socks, sand, and absorb mat to clean up the chemical left on the floor. To prevent the
surrounding environment, we need to put all of the containers in the safety place so that we can
manage it properly later and put used materials including socks, sand , and absorb mat in the
safety bin. And finally call the environmental protection service.
Situational Overview:
A chemical HAZMAT incident began outside near the football field and gymnasium at MUIDS. It
was a sunny day with the temperature at 37c, 50% humidity and 10km/hr wind. In the scene,
there were five different types of the hazardous chemical spills; 4 labeled including
polyurethane, toluene, sodium borohydride, tetrachloroethylene (Perchloroethylene) and
another one with no labeled. There were two victims laying on the ground in critical conditions.
One faced up and one faced down. Moreover, there were some random wooden pallets around
the victims and drains on the sides of the footpath and next to the football field. Two victims
were in critical conditions from the effects of hazardous chemicals.

Objectives:
- Rescue the people
- To remove the hazardous material spill
- Prevent the surrounding environment from getting damaged

Weather : Sunny and few clouds
Wind : 10 km/hr

Chemical barrels :
1.Polyurethane
● Properties:
- Flammables
- Carcinogen
- Irritant
- Resistance to water and oil
- Melting point: 57°C do not melt when heated

● Health effects:
- Cause asthma and other breathing problems(headaches! Coughing)

● First Aid:
-Inhalation- move victim to fresh air
-Swallow- can cause injury and death
-Eyes contact- run with water at least 20 mins
-Skin contact- Remove and isolate contaminated clothing and shoes
2.Toluene
● Properties:
- Very Flammables, -Carcinogen, Irritant
- Heavier than air
- Liquid will float on water
- Clear colourless liquid
-Boiling point: 110.6 °C
-Melting point: -95 °C

● Human effects:
-Toxic if swallowed or absorbed through skin
-Contact may irritate or burn skin and eyes
-Vapor cause dizziness or asphyxiation

● First Aid:
-Inhalation- move victim to fresh air
-Swallow- can cause injury and death
-Eyes contact- run with water at least 20 mins
-Skin contact- Wash with soap and water

3.Tetrachloroethylene (Perchloroethylene)
● Properties:
-Vapor form explosive mixture with air
-Vapor heavier than air
-Vapor explosion hazard indoor and outdoor
-Boiling point: 121 °C
-Melting point: -19 °C

● Human effects:
-Toxic if swallowed or absorbed through skin
-Vapor cause dizziness or asphyxiation
-Contact may irritate or burn skin and eyes

● First Aid:
-Inhalation- move victim to fresh air
-Swallow- can cause injury and death
-Eyes contact- run with water at least 20 mins
-Skin contact- Remove and isolate contaminated clothing and shoes
4.Sodium Borohydride
● Properties:
-React with water(flammable gases)
-May re-ignite
-Odorless white to gray-white microcrystalline powder
-Soluble in water
-Boiling point: 500°C & 1,388 °C respectively
-Melting point: 400°C & 318 °C respectively

● Health effects:
-Inhalation or constant can cause severe injury
-Contact may irritate or burn skin and eyes
-Swallow cause burn

● First Aid:
-Inhalation- move victim to fresh air
-Swallow- DO NOT make vomit. Run with water and take victim to the hospital
-Eyes contact- run with water at least 20 mins
-Skin contact- remove and isolate contaminated clothing and shoes

Equipment needed:
- Sand for tetrachloroethylene spill / sodium borohydride
- Suit level C
- Wood plug kit (block the chemical hole on the containers)
- Absorbent mat (absorb rhe chemical spill from on the floor)
- Nitrile or neoprene gloves(protect the hands)
- Socks( absor the chemical and to block it flowing in the drain)
Response Procedures:

1)Rescue people:

- Running their eyes with water for at least 20 mins, removing and isolating their
contaminated clothing and shoes and then washing their skin with soap and water. This
will clear the chemical out of their body at the very first stage before going to the
hospital.
- Call 911 and ambulance to notify them of any injuries sustained from the chemical spill
(state your name, location and chemical(s))
- Evacuate injured people to safety area (fresh air) and send to Golden Jubilee Medical
Center, Faculty of Medicine Siriraj Hospital, Mahidol University (the nearest hospital)
- Warn people in the surrounding area to be aware of the effects of chemical spill (use the
school announcement)

2)Remove harmful materials:


- Wear appropriate protective clothing, safety equipment (masks, goggles and gloves(suit
levelC))
- Use plug kit to block the chemical spill from the container
- Control the spread of liquid by using socks and sand
- Clean up the chemicals on the floor using sand and absorb mat

3)Prevent the surrounding/environment:


- Notify the authorities if a spill has a potential to cause environmental damage
- Put away the containers to a safety area
- Put away the absorbent material, socks and sand in the safety bin
- Call the environmental protection service
Conclusion :

In conclusion, after all the procedures are done. The area will be temporarily closed for
safety. Both of the victims will be safe but the hospital will still be tracking their health for further
information . Also, the student, teacher, and people around the school are safe from the
chemical because they are worn by the school announcement. The area where the chemical
spill will be cleaned up properly and will not remain any of the chemicals in the area or in the
drain. Lastly, all of the chemicals left in the barrels will be stored in the new containers and the
old one will be cleaned up properly by compatible solvent and put in the safety place in case it
can be reused.
