Activity Consent Form oReen” SEQ Schools Futsal Brisbane North 2020 STATE HIGH SCHOOL 22 June 2020 Dear Parent/Carer ur Football Excellence Program futsal squads will contest the SEQ Schools North Futsal Championships, in term 3; the first of which will be held at Boondall Entertainment Centre as part of the Brisbane North region. Your child has been selected to participate in this competition, subject to your permission. ‘The aim of the activity is to give students the opportunity to apply the acquired skills (from FEP sessions) in competitive game situations. Activity details: DATES: 42, 18 and 14 Boys — Tuesday July 28, 48 Girls and 15,16 Boys - Wednesday July 30 16 Girls and 19 Boys ~ Friday 31 July ‘VENUE: Boondall Entertainment Centre, ‘TIME: Departing: 7:30 am. Returning: approx. 4.30 pm cost: $13 ACTIVITY: Futsal (medium inherent risk level) played on an indoor court using regulation size balls and goals. ORGANISING TEACHER: Mr P. Freeman TRANSPORT: School organised chartered bus to and from venue. Parents, however, may elect to pick up their child from the venue. UNIFORM: School sports uniform (Including school bucket hat) needs to be wom at all times when traveling to the centre and in between games. FOOD: A canteen will be available at the venue; however itis adviseable that students organise their own food and drinks. ADDITIONAL INFORMATION: All students will be required to remain inside the centre for the day. ‘Students attending will need to make contact with their teachers, prio to the event, o discuss any work that they will miss, Please be aware that the organisers of the tournament have informed us that, due to COVID 12 restrictions, there will be no spectators allowed into the venue on any of the competition days. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may be also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to atrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.
