AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, LINO S. SAPUAY and YOLANDA A. GORDOLA of legal ages, Filipinos, and both
residents of Barangay San Antonio, City of Ilagan, Isabela, after having been duly sworn to in
accordance with law, hereby depose and state:

1. That we are the same persons executing this Affidavit;

2. That we personally know JONALYN BILANO GRABATO, since we have been neighbours
for a long period of time;

3. That she used the middle name “BILANO” on all her transactions and personal data
records;

4. However, upon looking at her Certificate of Marriage, her middle name was erroneously
registered as “VELANO” instead of “BILANO”;

5. We hereby certify and swear that the person with the middle name of “BILANO” and the one
with the middle name of “VELANO” which appeared on the Certificate of Marriage refers to
one and the same person;

6. That this instrument is being executed with no intention to defraud anyone;

7. That we are executing this instrument to attest to the truth of the foregoing and for
whatever lawful purpose that it may serve.
