Date: 02.05.06 Issued by: Designated Person ‘Approved by: Chairman of SMC DRUG & ALCOHOL POLICY Drug and alcohol abuse. and its effects, is one of the mast significant social problems and there have been a number of serious accidents to ships which have been directly or indirectly associated with the illegal use of such substances, including instances where the safety of ship itself has been jeopardised and the lives of the crew endangered. Ocean Tankers (Pte) Lid is committed to a safe, healthy and productive work place for all employees. The Company recognises that alcohol or drug abuse can impair one’s ability to perform properly and have serious adverse effects on the safety, efficiency and productivity of the employees and the Company as a whole. Therefore, a Company Drug and Alcohol Policy is implemented on all ship to provide a complete reference source for all malters arising from the ‘Company's rules on drugs and alcohol. As such: - 1. The misuse of legitimate drugs, or the use, possession, distribution or sale of illicit or un- prescribed controlled drugs onboard ship is prohibited 2. Possession, use, distribution or sale of alcoholic beverages onboard is specifically restricted and controlled. 3. Noaleohol should be consumed in the four hours prior to watch or duty. 4, A positive test result of blood alcohol content which exceed 0.04 gm / decilitre {gm/al) of blood or refusal to submit to a drug or alcohol test is grounds for disciplinary action including termination, 5. Regulation No, OT-TK-2-2 Revision, Now. 04 Title: DRUG AND ALCOHOL POLICY Date: 02.05.08 Issued by: Designated Person ‘Approved by: Chairman of SMC 4 TESTING PROCEDURES 1 1.2 1.3 24 22 23 34 32 33 34 35 36 The company requires crew to be submitted to drug and alcohol testing: 1 Prior to joining vessel 2 Unannouneed random testing to be conducted once a year per vessel for ail crewmembers, -3/ Monthly random alcohol test using breath alcohol-meter. 4 Where cause exists to suspect alcohol and drug abuse. ‘5 Post incidence testing to be carried out for any incident, which endangers life or safe operation of ship and risk environmental pollution. The company may also conduct unannounced searches for drugs and alcohol on Company operated vessels. ‘The drug and alcohol test records shall be maintained on board. GUIDELINES ON ALCOHOL CONSUMPTION ONBOARD. Blood alcohol content at any time while person is onboard shall not exceed 0.04 gm / decilitre (gm/dl) of blood. Daily consumption of alcohol per man shall not exceed 2 open cans (330ml) of beer in order to maintain the blood alcoho! content limit ‘Consumption of alcohol is strictly prohibited within four hours prior and during the watches and duties. RULES AND LIMITATION Max. one case (24 cans) of beer can be issued per person after every fifteen days and shall be recorded. The entitlement to purchase canned beer shall not be transferred or sold to another person onboard. No alcohol shall be allowed to be brought onboard from ashore by crewmembers or any other person. ‘Crewmembers on shore liberty must exercise restraint from excessive alcohol consumption in order to maintain their blood alcohol content while returning Any crew member, whether onboard or returning from shore liberty, with the sign of even partial impairment, can be tested to ascertain his fitness to stand duty, if found impaired shall not be allowed to stand duty. The matter is to be recorded in the official logbook and reported to the Head office. If the master is found impaired the chief officer shall report the matter to Head office. OCEAN TANKERS PTE LTD. No.: 04 Page: 3/3. | Tile: DRUG AND ALCOHOL POLICY Date: 02.05.08 | Issued by: Designated Person Approved by: Chairman of SMC 3.7 Inventory of canned beer carried by any crewmember onboard at any time shall not exceed 24 cans. 3.8 Pilots and navigation advisor on duty should not be offered any alcoholic beverage until completion of duty. 4 CONTROL AND MONITORING CHECKS 4.1 The master shall review the record monthly for compliance of the procedure and initial the record as checked, 4.2 The master and chief engineer shall randomly check the inventory of beer in the possession of crewmembers (max. allowable 24 cans per person). Excessive inventory may indicate possible violation of the procedure. 5 PENALTY FOR VIOLATION Any officer or crew found violating this procedure or the consumption limit prescribed herein shall be dealt with in accordance with the rules set out in the Company's Policy on aleohol and drug use and the employment contract, 6 INFORMATION ON ALCOHOL CONSUMPTION LIMIT 6.1 1 unit of alcohol on an empty stomach can raise blood alcohol content within one hour by 0.02 gavdt 6.2 2 units of alcohol during a meal can raise blood alcohol content by 0.02 g/dl 6.3 Alcohol metabolises out of the body at an average rate of 1 unit of alcohol per hour. 7 RELATIVE STRENGTHS OF THE DIFFERENT ALCOHOLIC BEVERAGES IN TERMS OF UNIT OF ALCOHOL. 1 can 330 ml (12 oz) of beer = 1 unit of alcohol. 1 glass (250 mi} of wine = 2 units of alcohol. 1 0z (28 ml) of hard spirits = 1 unit of alcohol. (eg. All departments shail co-operate and implement the decisions taken at this committee meeting during the discussions. Master may include other persons in the safety committee meetings, if necessary. Master will be chairman for both meetings. The Chairman should start the Meeting by reminding all those prasent, of the reason for and objectives(s) of the meeting. The Chairman should ensure that attendance is appropriate for the objective of the Meeting. (i.., no MORE and no LESS attendance than strictly needed) Chairman should establish an agenda The length of the meeting should be established in advance, and then adhered to, so as it does not cause hindrance to the routine duties of individuals ‘There should be a ‘critique’ or summary by the Chairman al the end of the Meeting. Minutes, or action points, should be as brief as possible and circulated preferably the same day but in any case within one week. 8 Meeting Reports The decisions taken at the meeting after discussions, shall be reported to the Superintendent. Technical on a monthly basis after the Master approves them on Shipboard Management Meeting form. A copy of minutes, Shipboard Management Meeting form, is to be maintained onboard. MASTER'S OVERRIDING AUTHORITY 2.1 Master has the overriding authority and responsibilities in decision making with regard to safety & pollution prevention, 2.2 The Master should not be constrained by commercial pressure of the ship owner's or charterer's when making such decisions. 23° Master may request Company's assistance as may be necessary. PROCEDURES TO IMPLEMENT COMPANY POLICY Master should implement the safety and environmental protection policy by conducting on board ing, interviews, drills, education and extending his co-operation and acceptance to all departments. ‘The Master shall have the responsibility for maintaining the Safety Management Certificate (SMC) issued by the Administration, PROCEDURES TO MOTIVATE CREW Master should motivate the crew in the observation of company safety and environmental protection policy, by regular monitoring of on board training, conducting interviews, enhancing the benefits of drills and by using all educational means available. Master may use both rewarding and reprimanding techniques as per his own judgement to motivate and realise the crew that safety is in their own interest. PROCEDURES TO ISSUE ORDERS Master shouid issue orders and instructions in a clear and simple manner, from time to time whenever he deems it necessary for the safely of life, ship and environmental protection. Such arders are to be signed by the Master and gerson in-charge of the operation. PROCEDURES TO VERIFY REQUIREMENTS Master should verify that specified requirements of safety management system are observed by all available means, e.g., by checklists available, random check and verbal confirmation during operations. Company will provide a checklist to the Master, to verify the implementation of SMS onboard. This checklist will be called “Vessel Audit Checklist (SMS)”. In order to determine that SMS implemented in the company and onboard the ship is easy to maintain, useful for everyone and able to ensure a real improvement in the safety and the prevention of pollution during operation of the vessel. 7.2 Master's review report will provide the company useful elements to best prepare the annual Management Review where decisions about policies and objectives are taken 7.3. The assessment must be a general overview basis and resul in information about the management of the full process, including Maintenance, Training, Inventory management, Safety at work, ete. with enough details to help the Company assess the level of priority. 7.4 The review is not as detailed as a ship audit, however itis taken from a higher point of view and refers to a whole part of the system. 7.5 The review will be conducted through the examination of documents such as: 1 Safety committee meeting records 2 3 External audit reports 4 Onboard training 5 Other inspections (e.g., Class, Oil Major & Port state etc.) 6 Technical notes 7 — Non-conformities, Accidents and Hazardous situations reports. 8 New legislation 9 New technology 10 Economic factors 7.6 — Master Review Report A report should be made by the Master after conducting a full assessment of the entire system, in conjunction with at least the Chief Engineer, Chief Officer and Second Engineer. Input from other officers should be taken into account Master will report back the completed review report to the to the Designated Person as per SMS document control procedures. Shall not leave the ship without the Master's permission. 2.4 — Shall return to the ship by the time specified by the Master. 2.5 Shall not use the ship's boat or its accessories without the Master's permission. 2.8 — Shall handie all toofs and items carefully and make efforts to conserve food and water. 2.7 — Shall not carry into or out of the ship portable items without completing the necessary formatities. 2.8 Shall not use unapproved electrical or fire equipment without the Master's permission and shall smoke at approved locations only. 2.9 — Shall not be involved in conflicts, get drunk or participate in violent activities onboard the ship. 2.19 Shall not exchange signals with shore or other ships without the Master's permission. 2.41 Shall not divulge confidential information about the company or work 2.12 Shall not engage in gambling or other similar activities onboard the ship 2.13 Shall not make use of the ship's provisions without the Master's permission and shall not present unnecessary bills for performing duties. 2.14 Shall not use items in ship's store, food and medicines belonging to the ship without the Master's permission. 2.15 Shall not raise animals on the ship without the Master's permission 2.16 Shall not invite unauthorised persons to visit the accommodation space, stay in the cabin, or use other facilities, nor carry items aboard the ship without going through format procedures. 2.17 Shall not engage in commercial transactions onboard the ship, nor take remuneration for labour outside the scope of duties. 2.18 Shall not possess dangerous weapons, explosives or other hazardous items. 2.19 Shall not engage in activities that may cause damage to hull, cargo, machinery or other equipment on the ship. 2.20 Shall not engage in activities that lead to disorder on the ship and bring dishonour to the company’s name. 3 Master's Responsibilities 3.1 Master to ensure that all shipboard statutory certificates are valid, company will maintain a list of expiry dates of certificates but in case no response has been heard from the company Master should notify the company in two month advance. 3.2 Master to ensure that class records are maintained on board in a proper manner. 3.3 Responsible for all services on the ship. 3.4.
