Refi Circular dated 03.08.2016 In continuation to the above referred circular and in response to the various queries from employees, the following consolidated guidelines are issued for Information /Compliance. I Serving/Retired Prime Beneficiaries Shall: a) Intimate the chenge of residential address to Establishment/ CHSS Section for updating the records. b) Get their claims endorsed only from AMOs to whom they are attached ©) Carry the CHSS cards along with them when they visit AMOs/Specialists/Labs/ Hospitals and produce before the Concerned Authorities on demand. d) Ensure while taking the referral from AMO to consult Specialists/Hospitals that all the columns are filled correctly and legibly viz,, Name, Centre/Unit, Staff No., Ward eligibility ete. Again verify at the time of submission. Tf not filled, they may fill the same before submission, ¢) In case of opting for higher ward, an undertaking letter may be given to Hospital specifying the reason for opting higher ward IZ Instructions/Information's: a) Submission of the annual declaration by the prime beneficiary is mandatory. b) Beneficiaries opting for higher ward during hospitalization/In-patient treatment in any of the CHSS recognized Hospital need to pay difference of higher ward charges and difference in the cost of procedures/treatments and other charges applicable for higher wards. ¢) Hospitals have been requested to allot only entitled wards to the beneficiaries. In case, entitled wards are not available, Hospital has been requested to allot higher wards at no extra cost till the accommodation is available in the entitled ward. d) The onus of ward eligibility is with the employee. e) Beneficiary shall avail treatment in Hospitals only for which the referral is taken. f) Admission/registration charges are neither admissible under CHSS, nor reimbursable. Hospitals have been informed that this is not chargeable, 9) As per MOU, only diet, telephone charges can be collected directly from the employees. Luxury Taxes are reimbursable.
