Application for Contractor's Final Payment Release Certificate

Description

This form shall be accomplished/filed by all contractors before the release of final payment by the Department of
Public Works and Highway (DPWH) in regards to contracts with the DPWH.

Filing Date

This form shall be filed in triplicate copies by the contractor thirty (30) days before filing a claim for the release of final
payment by the DPWH together with the necessary documents/schedules.
