FOR VALUE RECEIVED, the borrower, Roque Armentano Jr. promises to pay to the order of ROSEMARIE
CRIS FLORES the amount of P10,000.00 with the following conditions:

1. Amount will be due and demandable on June 20, 2021;

2. Principal amount shall have a monthly interest of 2%; and
3. Assignment is prohibited unless with prior consent of lender.

I, Roque Armentano Jr., agree and promise to pay to the order of ROSEMARIE CRIS FLORES the sum of
P10,000 for value received, with interest at the monthly rate of 2% payable after June 19, 2021.

If this Note is in default and is placed for collection, Roque Armentano Jr, shall pay all reasonable costs
of collection and attorney’s fees.
