1. I confirm that I have been given sufficient information about the study and the purpose of my
participation as an interviewee in this study has been explained to me and is all clear.
2. I confirm and understand that my participation is voluntary and there is no explicit or implicit
coercion whatsoever to participate.
3. My participation involves being interviewed by a researcher that will last approximately ___
minutes. I allow the researcher to take written notes during the interview also, the recording of
the interview through cellphone recorder.

Question 1: How does your organization affects your mental state now? Are there any changes over
time?

Question 2: Are you free of worry of stress?

3. Spiritual Being;

Question 1: How does your fraternity involvement affects your Spiritual life?
Question 2: Have you ever have a strong hope of having a good future ahead?

4. Social Belonging:

Question 1: How is your relationship with others? Are you having close bond with your family and
neighbors?

5. Practical Belonging:

Question 1: Are you currently working at a job? What kind of job do you have? If none, what makes
you busy?

Question 2: Do you find yourself a productive citizen ?
