Virtual Recording Consent Form November 2020
It is important that students of all ages consent to being a part of a virtual recording and are happy they are
included in the final video edit. All videos once recorded are submitted to a member of the editing, once the
video is complete all the submitted videos will be deleted. No individual videos will be kept on file for any other
purpose, they will only be used for the specific project you are requested to submit them for. It is also important
parents of students who are under 18 sign the form below. Only students who have their consent form signed
will appear in the final edit.
You might also be invited to submit a recording of you performing your own individual instrumental performance
for viewing on the Music School facebook page, for example similar to our 'Tune in a room' series we posted
during lockdown. Once you submitted video has been uploaded it will be deleted.
If you have any queries, do not hesitate to contact the Music School Office.
Please return to the Music School Office in advance of making your virtual recording
Please circle clearly: I am happy / not happy to give consent for my individual recorded video/performance to
be used as part of a project. I am fully aware the video will appear on Social Media channels and will be
deleted once the final edit is completed.
