AFFIDAVIT

I, RAQUEL T. LACHAONA, of legal age, Filipino citizen,

and a resident of Mayoyao Proper, Mayoyao, Ifugao, after
having been sworn to in accordance with law, do hereby
depose and state:

That I was the Barangay Treasurer of Barangay Mayoyao

Proper, Mayoyao, Ifugao, from December 1, 2010 to November
30, 2013;
That during said term, I was not employed in any other
position in the government and that I did not receive any form
of salary or compensation, except the honorarium for holding
said office, during my term of office;
Further, I am not related up to the 4 th degree of
consanguinity or affinity to the Punong Barangay Benjie P.
Kichayan of Mayoyao Proper, Mayoyao, Ifugao;
I am executing this affidavit to attest to the truth of the
foregoing and I am willing to affirm and confirm its contents in
any legal forum when required.
