Thank you for making an appointment for Angel at The Animal Medical Center with Dr. Schwartz. At The
AMC, we strive to make the appointment time as productive as possible for you and your pet. Therefore, we
prefer to obtain your pet’s medical records and previous test results in advance of your upcoming
appointment. If possible, please contact all veterinarians involved in your pet’s care and, most importantly,
those veterinarians who have most recently seen your pet for his or her current medical problem. Ask to
have the following information sent to your own address so that you may bring it with you to your pet’s
appointment at The AMC, or have the information sent directly to us via the e-mail address or fax number
listed below.
Please have the following information emailed or faxed to:
• client.services@amcny.org (preferred method)
or
• 212-752-2592
1. All diagnostic test results (blood testing, fecal testing, etc.)
2. All hand written medical records (particularly those related to the current problem)
Please bring with you to your appointment at The AMC:
3. A picture ID with your current address
4. All medications your pet has been prescribed and is currently taking
5. All Diagnostic Images*:
o X-rays
o CT (CAT SCAN)
o MRI
o Ultrasounds
o Echocardiogram

*Please Note: Viewing images is crucial to management of disease processes. As you may be aware, images come
in many formats. The following options are acceptable formats for providing images to The AMC:
• Bring a CD from your pet’s hospital (preferred)
• Bring radiographs from your hospital (they will be returned to your veterinarian upon completion of The AMC’s
use)

The Animal Medical Center’s Dr. Schwartz will contact your veterinarian to discuss the findings and potential
treatment plan after your pet has been examined and the records have been reviewed. Thank you for

Expectations during COVID 19
Dear Oreste Torres,

As an essential business, we have remained open throughout the COVID-19 pandemic for
your urgent and emergency pet health needs. Now, your pet can once again be seen by
our board certified specialists across more than 20 specialties and services, including our
community practice.
We want you to know that all recommended safety precautions are being followed as we
gradually reopen the facility to the public. For the time being, most appointments will
continue to occur via curbside drop-off and pick-up. On a limited basis, clients will be
permitted to enter the hospital following a health screening. Once inside clients must
maintain social distancing and wear a mask at all times.

• When you arrive at AMC you will be greeted by a facilitator at the front door. Clients
must come to the front door and speak with the facilitator to begin the check-in
process.
• After check-in, you and your pet can wait in our air-conditioned outdoor reception
area.
• Your doctor will call you on your personal cell phone to learn about your pet’s needs.
• When it’s time for your pet’s visit, an AMC staff member will take your pet into the
building.
• While your pet is at AMC, you are free to wait in our outdoor waiting area, in your
car, at your home, or anywhere that is most comfortable to you.
• On a limited basis, clients will be permitted to wait inside. Anyone let inside must
pass a health screening, practice social distancing, and wear a mask at all times.
• Once the visit is complete, you will receive a call to begin the checkout process.
• Following check-out, you will be notified when your pet is ready to be released and a
staff member will return your pet, along with any necessary medications, to you.

WHY CAN’T I COME IN WITH MY PET?
CURBSIDE CHECK-IN DURING COVID-19
We understand that you would like to be with your pet in the examination room

We enjoy having you there too.

It’s much easier for us to talk to you directly about your pet’s health.

However right now we are required to follow guidelines issued by New York State that state “Curbside
intake and pickup of animals should be used, wherever practical, to minimize the number of clients in
waiting areas and exam room.”

Why does NY State have such restrictive guidelines for animal hospitals?

The NY State government has put detailed guidelines in place for all businesses, not just
veterinarians. For example, medical facilities are required to screen patients before admitting them
into the facility.

Are these still necessary?


Restrictions are lifted on other things.

By July 2020, restrictions like curbside check-in have helped reduce the number of people diagnosed
with COVID-19 in New York State down from over 10,000 a day to below 1,000. At the same time we
have seen infections rise dramatically in other states who had fewer restrictions.

The government has lifted restrictions where it is essential to get other parts of the economy restarted,
but we don’t expect to see change for a while on safety measures like curbside check-in that have been
seen to be effective.

Let’s make this work together.

Curbside check-in is inconvenient for you and creates a lot of extra work for our staff. It’s hard on all of
us. We don’t get to see you like we used to.

But let’s work together to make your visit as easy as possible, using our curbside visit protocols.
