Pasient : Good morning.

Admission staf : Good morning

Pasient : I have an appointment with doctor puput today.

Admission staf : But at the first, you must sign up here

Pasient : Okay

Admission staf : Doctor puput is examining a patient right now. Please have a seat and wait a
momet.

Pasient : Ok. Thank you

Take 2

Nurse : Mrs. Ria buana

Pasient : here.

Nurse : here we are. What are you reasons for seeing doctor puput today ?

Pasient : I have a rash in arms and legs

Nurse : OK, let me take your vital signs

Pasient : How am I doing ?

Nurse : Everything is good. Normal blood pressure and no high temperature. Please wait
here for a minute. Doctor puput will be with you in a moment.

Pasient : OK, thank you.

Take 3

Doctor : Good morning, come on in and have a seat. Now what seems to be the problems?
Pasient : I have a rash on my arms and legs

Doctor : How long have you had the rash?

Pasient : It’s been about a three days ago.

Doctor : Are you taking anyting for it ?

Pasient : I put some cream on it but it does’t seem to be helping.

Doctor : I see. Are your allergic to any medications?

Pasient : Not that I know of.

Doctor : Im going to give you prescription for some ointment. I want you to apply it four
times a day. You should also avoid scrarching your skin. And its important to
use as little soap as possible. An appointment to see me next weeks if it doesn’t
geet better over the next few days.
