Contract No. 4600012931 ‘This Contracts made and entered into effective this 6th day of October, 2017, (‘Effective Date") by and between the City of Tacoma, a Municipal Corporation of the State of Washington (‘City’), and Diversified Maintenance Serves, Inc. (‘Contractor’). ‘That in consideration of the mutual promises and obligations hereinafter sot forth the Parties hereto agree as follows: 1 Contractor shall fully execute and diligently and completely perform all work and provide all services and deliverables described herein and in the items listed below each of which are fully incorporated herein and which collectively are referred to as “Contract Documents": 4. Request for Proposal - On Call Maintenance Services dated September 22, 2017 2. Contractor's submittal dated September 26, 2017 in response to Request for Proposal - On Call Maintenance Services. 3. City of Tacoma Insurance Requirements 4. General Provisions |The total price to be paid by City for Contracts full and complete performance hereunder may net exceed! $$ 90,000.00, plus applicable sales tax. Il Contractor agrees to accept as full payment hereunder the amounts specified herein and in Contract Documents, and the City agrees to make payments at the times and in the manner and upon the terms and conditions specified. Except as may be otherwise provided herein or in Contract Documents Contractor shall provide and bear the expense of all equipment, work and labor of any sort whatsoever that may be required for the transfer of materials and for constructing and completing the work and providing the services and deliverables required by this Contract IV. Contractor acknowledges, and by signing this Contract agrees, that the Indemnification provisions set forth in the controlling Contract Documents, including the Industrial Insurance immunity waiver (if applicable), are totally and fully part of this Contract and, within the context of the competitive bidding laws, have been ‘mutually negotiated by the Parties hereto. V. Contractor and for its heirs, executors, administrators, successors, and assigns, does hereby agree to the full performance of all the requirements contained herein and in Contract Documents. In the event the Phase 2 site is still operational after one (1) year, the contract may be extended for one (1) year upon mutual agreement of the parties. PURPOSE ‘To maintain all site facilities and services in clean, operable order. This scope includes inspection of the entire site facility and mechanical operations as well as providing maintenance service, repairs, and coordination of vendor services. It may also be necessary to perform repairs on or install new appliances and fixtures to existing facilities. SCOPE OF WORK ‘The selected maintenance services contractor will work with the City Point of Contact and Catholic Community Services, in maintaining, repairing, and responding to service calls for the complete site. 1. Perform weekly inspections, and the necessary preventative and corrective maintenance of the site, and all equipment and service trailers on the site, including but not limited to: a. Shower Trailer ~ including plumbing repairs, clear clogged or slow drains, etc. bb. Laundry Trailer — including performing normal maintenance such as inspecting dryer lint trap to clear of lint, make sure hoses are securely connected; however in the event of equipment failure, you would contact the equipment Owner for maintenance and repairs; c. Administrative/Security trailer; 4d. Medical trailer, e. Hand washing station(s); Sani-can(s); ‘Tent structure; Sunbelt Rentals cooling system(s); ‘Tent heater(s); Lift station and associated piping; Exterior lighting Electric panel(s) — visual inspection and confirm all breakers are set properly, removelreplace any blown fuses and breakers as necessary; Drainage system(s) — ensure all drainage systems are functioning properly; Encampment fence; Attached shed building; General cleanliness, trash dumped, facilities cleaned in accordance with janitorial provider's contract; Alll other utilities/services provided to the encampment facility; 1. Snow removal/de-icing services as needed. Must obtain pre —approval on de-icing products to be used, Fire Extinguisher(s) ~ inspect, replace as necessary; t. Pallet Shelters. PeRR Fret rsen 2 Respond to all requests for service within 24 hours. If the issue is a minor repair costing less than $500, then selected maintenance provider may proceed with the repair and provide notice of the repair to the City Point of Contact. If the issue is a major repair where the cost is estimated to exceed $500, or requires replacement of equipment that does not have an existing service agreement or contract, then selected maintenance provider shall prepare a scope of work for the repair or replacement including a cost estimate. The cost estimate may include a bid from a subcontractor if needed. This scope and bid/estimate shall be provided to the City Point of Contact. Once scope and budget is approved, the selected maintenance services contractor shall repair or replace items as described in the approved scope. If the issue is with equipment/trailer where the Owner provides their own service on (such as the laundry equipment/trailer or the sani-cans), then the selected maintenance provider shall contact the provider and submit a request for service, request estimated time of arrival, and follow up to confirm that issue was resolved and facility is back in service. Coordinate vendor services and communication with the designated City of Tacoma contact. ‘There are already several vendors providing regular services to the encampment (Catholic ‘Community Services, Janitorial services, sani-can services); we need one central operations point of contact. After each significant weather event, i.e, heavy rainfall, snow, freezing temperatures, perform a site inspection/assessment of conditions and respond accordingly to resolve any issues found as necessary. 6. Qualifications to include plumbing, electrical, mechanical proficiency, as well as general handyman skills. 7. See attached copy of initial assessment report for example of types of issues to identify, address, and resolve. PROJECT MANAGEMENT ‘The maintenance services contractor will generally coordinate his/her own work and shall provide qualified superintendence. PREVAILING WAGES REQUIREMENT Ataminimum, prevailing wages shall be paid for all labor supplied under this maintenance service contract. The contractor shall be required to post on the job site a copy of the Intent to Pay Prevailing Wages form. On an annual basis or as required, prevailing wage labor pricing will be adjusted to reflect any changes that have occurred during the term of the contract. PAYMENT AND PERFORMANCE BOND ‘The Respondent to whom the Contract is awarded shall provide a payment and performance bond, including power of attorney, for 100 percent of the amount of his/her bid (including sales taxes), GENERAL PROVISIONS The City of Tacoma General Provisions shall apply. Diversified Maintenance will maintain communication with City of Tacoma point of contact as well as work with Catholic Community Services to identify and resolve any ‘maintenance issues that arise on the site. Repairs under $500.00 will be completed and billed separately on a ‘Time and Materials basis. Sub-contractors, if used, will comply with Prevailing Wage requirement. Diversified Maintenance Service, Inc. will provide service as detailed below. Complete a site inspection one (1) time a week for a monthly rate of $975.00 + Tax. Complete a site inspection two (2) times a week for a monthly rate of $1,950.00 + Tax. Complete a site inspection three (3) times a week for a monthly rate of $2,925.00 + Tax. We are able to respond to additional requests for service within 24 hours. Additional hours billed separately. Additional hours of service are billed as detailed below, 2-hour minimum. Regular hours of service Monday ~ Friday 0600-1800 $75.00 + Tax per Hr. After hours of service Monday ~ Friday 1800-0600 $112.50 + Tax per Hr. Weekend and Holiday service $150.00 + Tax per Hr.
