Schools Division Superintendent SUBJECT SUBMISSION OF CAREER GUIDANCE PROGRAM FOR GRADES 10, 11 AND 12 MONITORING REPORT FORM DATE February 7, 2019 Particulars 1, Asper DepEd Order No. 41, s. 2015, DepEd Memorandum No. 165, s.2017 and DepEd ‘Memorandum No. 169, s.2018 on the Implementation of Grade 10, 11 and 12 Career Guidance Program, respectively, the school heads and the youth formation coordinators shall ensure and supervise the implementation of career guidance modules. 2. The school guidance counselor/designate shall accomplish the Monitoring Report Form provided in the above mentioned DepEd Memoranda. The report shall be submitted to the Youth Formation Unit of the School Governance and Operations Division (SGOD) for consolidation on the following dates: Career Guidance Program Dates Grade 10 (3 modules) On or before February 22, 2019 Grade 11 (8 modules) On or before March 8, 2019 | Grade 12 (8 modules) On or before March 22, 2019 3.
