Adrenal Fatigue Symptom Tracker

Use this worksheet to keep track of your symptoms during your treatment for Adrenal Fatigue. Simply enter your score in each of the
boxes and add up the total. As you start to feel better, this should be reflected in lower scores each week. Use the chart on the last
page to monitor your progress and stay motivated. You can also show this worksheet to your doctor when discussing your symptoms.
Note: Answer each question with a score from 0 to 10 (higher score = worse symptoms). Complete one column each week.

plot it on the chart. If you are following our recommendations on diet, lifestyle, supplementation and more, you should be feeling better
and your score should be dropping. If you connect the plot points, you should see a line trending downwards from the top-left to the
