Parental Consent for Individual Child Observation

I give my permission for my child, ________Erika Norseth_____, to be the focus child

for __Emily Adam_, a student in the Brigham Young University-Idaho CHILD 350 course, for

the purpose of learning about observation and assessment of child development and learning.

During this time, the above-named student will perform formal assessments and observations of

my child. I understand that my child’s name and other personal information will be kept

confidential and all the information gathered will be used only for the purposes of assignments

for this course. I understand that all information will be available to me and that I may ask

questions at any time. At the end of the semester the above-named student will share what they

have learned with me. I further understand that I may revoke my consent in writing for any

- This one’s name is scary pet, and the second one’s name is harmony. One’s a beanie boo
and the other is a feisty pet.

- Mom says you just open a can of worms. Erick Ya, I want to show her the toys in my

- Erick is at a normal social devleopment for an 8-year-old. She enjoys being around her
friends. He mom says she has a hard time when they have to leave.
- Erick is obsessed with miniatures.
- She has a barbie she loves that is named Ericka. She Said’s its her favorite toy.

Parent Letter

Interviewing Ericka was a treat. She seems like a lovely well-rounded girl. At first was a
little shy but with mom was fine. My first question about her toys was about her unicorn’s toys, I
asked her what their names are? She talked about how one’s name is scary pet and the other is
named Harmony I could not help but giggle at the scary pet, when you squeeze it. It becomes
scary, first glance it is not. Ericka is good at speaking to me with no hesitation and is at a normal
level of development for social interactions for an 8-year-old. After that we talked about the
different toys she has, and can I see more toys? She like Yes of course look and we Went to see
the toys in her room. Ericka is such a great communicator about what she wants to do. But
before her room we stopped at her room we stopped at the kitchen to see her cupcake dolls. She
talked about how she has 3 a large one a medium and a small one. I asked her if she likes to play
with them with her sister Gigi. She says yes Gigi has 2, She has a small one and a large one. She
then led me to the bedroom, and she showed me her favorite loft bed and how she loves to play
under it. She also showed me Ericka her barbie. We talked about how they have the same name.
so, what does this all mean. Erika is at a normal devleopment for an eight-year-old socially. She
loves to play with her friends she enjoys’ s time with her parents and loves to play barbie’s. She
is a little shy at first but once she is next to mom is all open with anyone. I have enjoyed the time
I had with her thank you.
