English is an international language that is used almost all over the
world, in only how to use the question word 5w + H correctly according to its
use, for example the use of the word what is used to ask for objects, activities
or opinions, the word when is used to ask when and time, the word why is
used to ask the reason or cause of an event, the word who is used to ask
people, the word where is used to ask about the place of the incident or the
place of activity, how is used to ask about conditions, circumstances, events or
activities.

In the world of health the use of question words 5w + H is also very

much needed in assessing patients to obtain patient data through systemic
methods used to identify strengths, weaknesses, physiological status,
knowledge, motivation, support systems, and client coping abilities that can
affect client's health both positively and negatively

History talking is done to collect subjective data through interviews with

patients. In collecting patient data, we must first collect nursing history
assessment which consists of data administration, no Reg, hospital, time, and
medical dx. After that, we look for patient data including name, age, sex, race,
religion, education, ocupation, address. After knowing the patient's identity,
then we look for family health history to find out whether the disease suffered
by the patient is a hereditary disease or not.

After collecting all the data from the patient, then we perform a physical
examination which includes inspection, palpation, percution, and auscultation.
