AFFIDAVIT of NO RENTAL

I, Mayflor Villamor, of legal age, single, Filipino citizen and resident of 16N Escario
St., Kamputhaw Cebu City, after having been duly sworn in accordance with law, hereby
depose and state:

1 That I am the nephew of the property owned by Uldarico Y. Alviola which used as by my
office for my trading business located at 16N Escario St., Kamputhaw Cebu City.
2 That ALLPRO CONSUMERS GOODS TRADING had currently functioned to the said property
due to its purpose as office only and at the same time as my residence.
3 That ALLPRO CONSUMERS GOODS TRADING is not renting any premises to operate business
considering that only to utilize space.
4 That I am executing this affidavit in compliance for application of BIR registration, business
permit requirements information update and to show that I am not renting any place for
business and for whatever legal purposes this may serve.
