2021 Quartz Well Points for Diabetes Self-Care

Quartz will provide up to 1,000 Quartz Well* points to members with diabetes who complete recommended
tests and screenings. Optimizing your health as you live with diabetes can pay off when you redeem Quartz Well
points for Amazon gift cards!
Tests and screenings that earn points include:
 Hemoglobin A1c – two tests at 200 points each, up to 400 points per year
 Estimated glomerular filtration rate (eGFR) – 200 points
 Urine to albumin creatinine ratio (UACR) – 200 points
 Eye exam that includes evaluation of the retina – 200 points
There are many other ways to earn Quartz Well points, like by syncing your fitness tracker or taking an online
workshop. You can check your points level in Quartz Well from your MyChart account, And, when you reach a
qualifying level, be sure to claim your gift card.

*Due to overlap with the Well Wisconsin Program, the Quartz Well reward program is not available to State of Wisconsin Group Health
Insurance Program members.

Subscribers age 18 and older can earn $100 for single plans. Family plans offer $100 for the subscriber and $100 for the subscriber's
spouse (or domestic partner).

Quartz Well points can take up to 90 days to be awarded. Please allow claims-processing time for points to be added to your portal.
