Twiggs Call Center Agency prepared this safety report for the ongoing renovation and inspection of the
establishment. The inspectors and the construction team reported a gas leak on September 5, 2025, near the
pantry rooms.
Objectives
Incident Information
Ms. Jennifer Linn, the building safety o cer, was informed by the construction workers about the evident smell
of gas coming from the pantry rooms. Fortunately, there was no one using the rooms, and no one was using
items that caused a re when the gas leak was detected.
Methodology
The team gathered facts about the incident from interviewed employees in the agency and supervisors. Copies of
the safety protocols, currently followed by the workers, were also collected to conduct the investigation.
1. The pipes intended for the small cooking range in the pantry were not tted or may have been moved during
the renovation.
2. An employee of the agency used the range but failed to notice the leak.
Conclusion
The stated incident happened due to the failure of the safety o cer to remind the workers about the connections
of the pipers and other elements of the establishment.
Recommendations
1. The pipes should be moved to a more stable and higher place to avoid having the same incident occur again.
2. There needs to be a security guard roving the pantry rooms to check on the appliances and the conditions of
the items in the rooms.
