The Columbus, Ohio, Chapter 13 Trustee's Office now offers Electronic Funds Transfer (EFT) as a
payment option during our monthly disbursement process. This will consolidate our disbursements to you
into one direct deposit, enable you to receive payments on your claims more quickly, and eliminate the
possibility of checks being lost in the mail. To begin the process to receive EFT payments at our monthly
disbursement, please complete and return the form below to us at our correspondence address of Chapter
13 Trustee, Edward A. Bailey, 130 East Wilson Bridge Road, Suite 200, Worthington, Ohio 43085-
6300. We will send you the required forms for account verification. EFT payments receive an email
voucher and also are able to access the transaction detail for EFT payments via our website.
