QUALITY ASSURANCE, TESTING AND INSPECTION CONTRACTOR'S QUALITY ASSURANCE The Contractor shall establish and operate throughout the execution of the Work, a Quality Assurance system complying with ISO 9001. The Quality Assurance Plan (QA Plan) established and operated by the Contractor shall be independent of any Quality Control (QC) or Quality Assurance programme or schemes which may be implemented by the Engineer. Specifically, the Contractor shall prepare a QA Plan for the Work which shall be submitted to the Engineer for approval within 8 weeks of the award of the Contract. The approved QA Plan shall be reviewed, updated and re-submitted for approval as necessary throughout the execution of the Work. The QA Plan, including a fully detailed Inspection and Testing Plan, shall specifically address the procedures for maintaining the project requirements with respect to the use of subcontractors, vendors, suppliers, the Contractor’s own personnel and any Independent Inspection Agency and shall reflect the quality of the item or material concemed. The criteria for the assessment of quality shall be as approved by the Engineer. ‘The Contractor shall keep a detailed record of all quality control and quality assurance inspection and audit activity. The said records shall identify the inspector, the place, the date and time the inspection ‘was completed, the section of the Works inspected or part of the trials and its state of completion. Reference shall be made to the relevant detailed drawings and specification(s), and the specific aspects, or properties which were checked or measured shall be recorded. The records of inspections and tests shall be stored in an orderly fashion at the Site by the Contractor until the issue of the Performance Centficate for the whole of the Works, or such earlier time as the Engineer may instruct, and the Engineer will have the right of access to these records always. ‘The Contractor shall cooperate with the Engineer and provide all necessary access to the works, testing laboratories and records to enable the Engineer and the Employer to assess the Contractor's Quality Assurance system and to audit the implementation of the QA. Plan and associated procedures. The Contractor shall provide saraples for testing or inspection, as and when directed by the Engineer. After the issue of the Certificate of Completion for the whole of the Works, or such earlier date as the Engineer may instruct, the Contractor shall deliver the inspection records to the Engineer as directed TESTING AND INSPECTION BY THE ENGINEER In addition to any testing and inspection carried out by the Contractor under his Quality Control (QC) and Quality Assurance (QA) programme, the Engineer may usdertake his own testing and inspection of the Works, In this regard, the Contractor shall afford and facilitate access at all times for the Engineer’s inspection and testing of materials and workmanship. The Contractor shall provide means of access and alt assistance as may reasonably be required by the Engineer. For all items of work, the Contractor shall give the Engineer at least 24 hours written notice, informing that the item in question is complete and is, ready for inspection, and the Contractor shall not cover it up by subsequent work until the Engineer has, been given adequate opportunity to carry out any necessary testing or inspection. In case where the preparation or manufacture of any component of the Work or part thereof is to be carried out at a location remote from the immediate Site, the Contractor shall give specific written notice to the Engineer. Such notice shall state the place and time of the preparation or manufacture, quarrying or extraction and shall be given sufficiently in advance of the activity as to enable the Engineer to make such staffing arrangements as he may deem necessary for its inspection at any point in its preparation or ‘manufacture. Works to be carried out in a location remote from the immediate Site as aforesaid shall not Commence without the prior, written approval of the Engineer. ‘Any component of the Work or part the:eof which are prepared or manufactured without such prior notice having been given to the Engineer may be rejected if the Enginecr considers that his inspection was necessary during the preparation or manufacture ofthe component or part thereof. No testing, inspection or approval by the Engineer, of any item of the Works shall relieve the Contractor of any of his duties or obligations under the Contract, in any way. MAINTENANCE MANUAL AND “AS-BUILT” DRAWINGS Pursuant to Sub-Clatise 4.1 [Contractor's General Obligations] subparagraph (d) of the General Conditions of Contract and before the completion of the Work, the Contractor shall submit to the Engineer four hard copies and two soft ccpies of an Operation and Maintenance Manual in the Bangla and English languages. ‘The Operation and Maintenance Manual shall include, but not be limited to, the following; (a) a folio or record of “As-built” Drawings showing all details as constructed for the whole of the ‘Works which shall be in English language only; (b) in addition to sections relating to civil, structural constructed items there shall be discipline specific sections for Architectural related items, as well as Track, Bridge ete.; (©) operating instructions applicable to any items of mechanical or electrical equipment or appliances, provided by the Contractor or his subcontractors or suppliers; (@) a schedule for the inspection and monitoring of all component parts susceptible to wear and tear, damage or weathering and deterioration with time; (©) @ schedule of routine and periodic maintenance activities appropriate for the climate, the anticipated level of use or exposure and the availability of skilled labor; (f) instructions and procedures for the correction of faults or foreseeable accidental damage; (8) specifications for materials and workmanship for all predictable routine and periodic maintenance or repair activities; and (h) lists of tools, instruments and equipment necessary for all inspection, operation and maintenance activities. The Contractor shall submit “drafts” of the Operation and Maintenance Manual for approval not less than eight weeks before the date fixed for the commissioning or handover of the completed Works. Any modifications to the form or content of the Maintenance Manual as required by the Engineer shall be incorporated in the final issue of the Maintenance Manual There will be no separate payment for the provision of Operation and Maintenance Manual and “As- built” drawings. The same cost shall be considered as being included in the rate of the items of work in the Bill of Quantities. INDEPENDENT INSPECTION AGENCIES Where the Contractor is required to ensure that quality assurance and quality contro! measures are carried out in focilities that are not near the vicinity of the Project, the Contractor will engage the services of an Independent Inspection Agency (IIA). Under these circumstances, the Contractor shall propose more than one and preferably at least three (3) alternatives, for the Engineer's Approval, either per Country or per specialty or by any other criteria as judged technically or commercially appropriate by the Contractor. The Contractor's proposal shall contain clear reference to scope of services to be provided, qualifications and experience of the Inspection Agency in general and the qualifications and experience of the personnel who will be designated as responsible for conducting the requisite inspection and test procedures.
