AFFIDAVIT OF HEIRSHIP

I, CHANYEOL A. PARK, of legal age, Filipino and a resident of Davao City,

Philippines, after being duly sworn to in accordance with the law depose and say:

That I am the heir of the late MAMA A. PARK, who is the registered owner of a certain
parcel of land situated at ULA, Davao City with an area of ________ (_____) SQUARE
METERS more or less and more particularly described as Lot 154-M, Psd-11-01915 covered by
Transfer Certificate of title No. T-158845.

That this affidavit is executed in order to appraise the government authorities concerned
and does not come under the operation of the Land Reform Code and for whatever legal
purposes it may serve best.
