ACCOMPLISHMENT REPORT
APRIL 2021

The following are my accomplishment report:

Duty at the Bingawan Boarder every Monday and Tuesday.

Monitored the LSI with BHW and DCW every Wednesday.
Assisted the BHW and DCW to monitor the temperature of individual who are in home
quarantine every Wednesday.
I and the DCW go to Barangay Quarantine Facility to check the quarantined person every
Thursday.
I and the DCW went to the TNHS Quarantine Facility to check the temperature of the
LSI every Friday .
I, together with the BHW and DCW started to collect the data that the ICS needed.
In the following days, we visited the home quarantined persons to check their body
temperature and to check also if there is a symptoms occur.
