To whom it may concern:

This is to authorize my wife, JENNEFER M. FERRIOLS, to retrieve a new password

for my SSS account that has been blocked for unknown reasons. I authorize my wife
to do this action in the SSS-Digos branch especially since I need to log into my
account and the block has deemed me unable. I have enclosed a scanned copy of my
ID as further proof of my authorization.
