APPOINTMENT FOR DRIVING FITNESS

In view of Covid-19 Pandemic, this institute has made provision of giving driving fitness
appointments through email.
Persons with Locomotor Disability seeking appointment for driving motor vehicle fitness are
requested to apply by sending an email on aiipmr.msw@nic.in .
Following details should be submitted while seeking appointment-

 Full Name
 Gender
 Date of Birth (dd/mm/yyyy)
 Address
 Contact no (mobile number preferred)
 Email address
Also attach a scanned copy of: -

 A recent Passport size photo (3 copies of the same photo should be carried at the
time of appointment)
 Disability Certificate (taken from this institute /any Civil hospital)
 Residential Proof (Passport/Driving License/Voter’s ID card/Electricity bill or phone
bill)
 RTO referral letter (only for the people residing outside Mumbai district)

Your date of appointment will be conveyed on your email address.

On the day of appointment, you have to bring all the above documents in original with
photocopy/xerox of each document, also carry:
Photo ID, issued by any State/Central Government authority.
Medical reports pertaining to the disability
Note:

 No request for pre-ponement of the appointment will be accepted.

 Persons with multiple disabilities along with locomotor disability (Visual
Disability/Hearing Disability/Speech Disability) need to submit fitness to drive
certificate from the respective authorities on the date of appointment.
