HEALTH DEPARTEMENT POLITEKNIK NEGERI MADURA TAHUN 2021 GERIATRI HEALTH DEPARTEMENT Geriatrics in medical world is a branch of doctors science that focuses on the treatment, diagnosis and prevention of diseases and health problems that affecting elderly patient. Geriatric department is a treatment in health aspect for elderly patient and examine with promotion, prevention, diagnosis, medication, and rehabilitation. Elderly patient always get some problem in vision quality, hearing, easily get tired, loosing acuity of mind, even diabetes and heart desease. In the fact elderly patient need special treatment that different with adult patient, that’s why their treatment is more complicated. Moreover elderly patient always have more than one desease for examples heart desease, diabetes, arthritis, Alzheimer, or hypertension. This condition makes health treatment for elderly people be complicated and that’s why elderly patient need treatment to handle the conditions with geriatric doctor. Geriatric nurse is one of important part for patient, especially elderly patient. Geriatric nurse will help patient to do mobilitation, and elderly patient life’s quality. They are trained to anticipating elderly’s requirement and collaborate with doctors of family to giving individual treatment in intensely. Various conditions and diseases in the elderly can cause memory problems, difficulty holding back bowel movements, body weakness, or experiencing health problems due to the side effects of certain medications. Not infrequently, the elderly have difficulty carrying out daily activities, including eating, bathing or dressing. The role of elderly nurses is to help manage these conditions. Geriatric doctors and geriatric nurses generally understand that the care of the elderly needs to be done thoroughly. Starting from evaluating the disease that has been suffered to the right treatment plan in collaboration with the family. To take serious Efforts to improve the welfare of the elderly are directed at extending the life expectancy and productive period in order to create independence and prosperity. One of the efforts is improving geriatric health services in hospitals. as an effort to improve geriatric health services in hospitals for quality, equitable and affordable, geriatric services must be carried out in an integrated manner through an interdisciplinary approach by various professionals working in an integrated geriatric team. To give the various specificities of travel and disease appearance in the elderly, there are two main principles that must be fulfilled in order to carry out health services for the elderly, namely a holistic approach and team work and management. Geriatric patients are handled by the Integrated Geriatric Team; is a multidisciplinary team that works interdisciplinary to deal with elderly health problems with the principle of integrated and complete service governance by bringing services closer to elderly patients. The workforce in Geriatric services consists of health workers and non-health workers who work together as an Integrated Geriatric Team. The Geriatric Integrated Team consists of a chairperson and a service coordinator who is also a member and a member. The Chairman of the Integrated Geriatric Team consists of doctors who specialize in internal medicine. Geriatric services are provided to elderly patients with the following criteria: having more than 1 (one) physical and / or psychological illness; or having 1 (one) disease and experiencing disturbances due to decreased organ function, psychology, social, economy and environment requiring health services. Apart from elderly patients over 60 years; Geriatric services are also provided to patients aged 70 (seventy) and over who have 1 (one) physical and / or psychological illness. The types of services is provided to Geriatric Patients; among others, are as follows: Types of Geriatric services at a simple level consist of at least outpatient and home care, types of complete Geriatric services at least consist of outpatient care, acute inpatient care, and home care, Types of services At least perfect level of geriatrics consists of outpatients, acute inpatients, home visits, and day care clinics, full-level geriatric services consist of outpatients, day care clinics, acute inpatients, chronic inpatients, and inpatients. Psychogeriatrics, Geriatric Patient care (respite care), home visits (home care), and Hospice. All of poly/department in hospital will always get conflict especially in nursing group, because nurse is someone who’s almost 24 hours with patient. Conflict that always appears in geriatric nurse group is about informed consent. Some of patient’s family are too indifferent, the reason that they always use is they are too busy in work world or with their new family. So the decission to patient’s recovery will always get delayed. In nursing station we always get a conflict about what decision that we have to choose, is it obeying the standard operational procedur or we have to prioritizing patient’s safety. At the end decision that we always takes is we still obeying SOP but patient still in safety area. Geriatric nurse will trying so hard to get informed consent from patient’s family either we call our friends or maybe family who’s in the same area with patient’s family location and then talking to them by via telephone because they drop non-active telephone number and didn’t reconfirm the new one to nurse station. Some of them is still too busy and can’t come to the hospital when we ask to explain patient’s conditions and of course we have to explain patient’s condition and health education about the risk and the health procedur that we want to take by via telephone and then record it. Giving treatment for geriatric patients is a glory. There are lots of new things happening, lots of things we have to discuss together in the nursing station, and we will always take the best decision for our patient.
