REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City MEMORANDUM To: All Revenue Officials and Employees SUBJECT: Coronavirus Disease (COVID-19) Vaccination Survey DATE: May 7, 2021 ‘The BIR continues to support its employees during this pandemic while encouraging everyone to exercise their social responsibilty of helping mitigate the spread of COVID-19 in the country. As the exigencies of the Bureau's mandate requires us to continue normal work operations, the safety and security of ali employees and taxpayers remains our top priority. As vaccination efforts spearheaded by the Inter-Agency Task Force for the Management of Emerging Infectious Diseases (IATF) continue to evolve, the Bureau has requested an onsite COVID-19 Program in line with |ATF Resolution No. 110 dated April 15, 2021 to encourage and support employees who choose to receive the vaccine when they become eligible. The resolution states that frontline government workers in charge of tax collection are included in priority group 4.15 of the National COVID-19 Vaccine Deployment Plan To materialize this initiative, the Bureau needs to gather actual figures of interested employees to receive the COVID-19 vaccine. In this regard, all heads of offices in the BIR National Office and Regional Directors thru Chief, Administrative Human Resource Management Division in the Regional Offices are required to submit (in hard and soft copies) the attached COVID-19 Survey with responses from all the BIR personnel under their jurisdiction.
