Are you a Black or African American parent or caregiver of a child with special health care needs or disabilities?
Are you interested in sharing your experiences with accessing care, interacting with health professionals, and advo-
cating on behalf of your child/family?
Are you willing to share your insights and perspectives on building effective relationships with pediatricians?

The National Resource Center for Patient/Family-Centered Medical Home (NRC-PFCMH) wants
to hear from you!

When and Where: Saturday, July 10, 2021 from 10:30am– 12:30pm CT. The NRC-PFCMH staff will share the Zoom link
closer to the event.
Requirements: Must have reliable access to the internet and a web camera.
All participants will be paid for their time and participation.
