JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JUAN DELA CRUZ, of legal age, Filipino, and a resident of _________,
Philippines, and JANE DELA CRUZ, of legal age, Filipino, and presently residing
at _____________, Philippines both after being duly sworn to in accordance with
law, depose and state that:

1. We personally know PETER DE PEDRO of ____________________, and

JOHN DE PEDRO of __________________, being an acquaintance for a
long period of time;

2. We declare, confirm and affirm that PETER DE PEDRO and PETRA DE

PEDRO are the biological and lawful parents and/or guardian of JOHN DE
PEDRO who was born on September 30, 1996 at Western Visayas
Medical Center located at Q. Abeto Street, Mandurriao, Iloilo City,
Province of Iloilo, Philippines;

3. The PETER DE PEDRO that we know and the PETRE DE PEDRO

indicated in the Certificate of Live Birth of JOHN DE PEDRO are one and
the same;

4. Also, the PETRA DE PEDRO that we know and the PETAR DE PEDRO
indicated in the Certificate of Live Birth of JOHN DE PEDRO are one and
the same;

5. We are not related by affinity or consanguinity to the above-mentioned

persons; and

6. We are executing this affidavit to confirm and affirm the foregoing facts
and for whatever legal purpose it may serve.
