PAST HISTORY OF ILLNESS:

 Past history includes all significant illness and hospital admission from infancy to the
time of presentation .Past history includes the following points.
 The attack of same diseases in past?
 Any history of relevant diseases especially in the case of childhood.
 Any history of viral disease during pregnancy
 History of difficult labor .History of child birth
 Any history of acute disease in previous 5 to 6 months.
 Any history of chronic disease with its actual time of that disease.
 Any history of trauma, burn, accident.
 Any history of infectious disease like mumps, chicken pox.
 Any history of warts or moles, itching, eruption, skin disease.
 Any history of vaccination, X-ray, snake bite e.t.c.
 Any history of venereal disease( Mode of treatment)
 Any history of suppression.
 Past history may b important due to diagnosis of present illness
 Irregular treatment of Pulmonary Tuberculosis in lungs diseases
 Abdominal operation in intestinal obstruction
 Jaundice in case of Cirrhosis of liver
 Hypertension and Myocardia in faction in Congestive Cardiac Failure

Drug history:
 It gives us clue for any chronic disease which may responsible for present illness.
 Inquire about name of drug, its dose and repetition.
 Any use of alternative treatment, its name, dose and repetition.
 Ask the patient to bring previous prescription.
 In case of Diabetic patient inqure about the taking drug regularly or as prescribed
 Any history of drug allergy e.g nausea vomiting. Pencilline ,digitalis etc
