I, ___________, of legal age, Filipino citizen, and a resident of No.40 Km. 4,

Asin Road, Baguio City, after having been duly sworn to in accordance with law, hereby
depose and state that:

1. I am the attorney-in-fact of plaintiffs-appellants in the above-entitled case.

The copy of my SPA is duly attached in the records;

2. I have caused the preparation of the foregoing appeal memorandum and that
I have read and understood the same and hereby verify that the allegations in
the pleading are true and correct based on my own personal knowledge and
on authentic documents;

3. That the pleading is not filed to harass, cause unnecessary delay, or needlessly
increase the cost of litigation;

4. The factual allegations therein have evidentiary support or, if specifically so

identified, will likewise have evidentiary support after a reasonable
opportunity for discovery;

5. That I hereby certify that I have not commenced any other action or
proceeding involving the same issues and parties to any other court, tribunal
or quasi-judicial agency and to the best of my knowledge, no such other action
involving the same issues and parties or proceeding is pending in any other
court, tribunal or quasi-judicial agencies, the Court of Appeals or Supreme
Court;

6. I hereby undertake to notify this court within five (5) calendar days from
notice should I learn that a similar action or proceeding involving the same
issues and parties has been filed or is pending before any court or tribunal or
quasi-judicial agencies the Court of Appeals or Supreme Court;

7. That I hereby execute this verification and certification in compliance with

Rule 7 and Rule 37 of the Amendment to the Rules of Court.
