Patient name: Ray Nelson

Age: 4 years old

Clinical presentation:

The patient has down syndrome

He is presenting with cyanosis, shortness of breath, most noticeable during feeding and play time,
and agitation

Poor weight gain

Physical examination:

On auscultation, a systolic murmur on the left lower sternal border was noted

Oxygen saturation was 85%

Diagnostics

Ultrasound of the heart shows intra ventricular septal defect, with a hypertrophied right ventricle

And aortic dextraposition, an stenosis of truncus pilmponalis

Diagnosis

Based on the presenting symptoms and the echo cardiogram ndings, the patient has tetrology of
fallot

Management

Oxygen therapy with iv uids

Treatment

Intracardiac repaire, with widening the pulmonary valve and enlarging the passage to the
pulmonary artery
