Grade 10 students,

For those who will have their Grade 11 in private schools, you have to apply for SHS voucher so that you can receive a
subsidy from the government.

Senior high school voucher should be applied by the students (voucher applicant) together with the guidance of their
parents. The adviser will orient you on the process of the application.

As of the moment, SHS Online Voucher Application Portal for incoming Grade 11 students of SY 21-22 is not yet open. 
You may check it at http://ovap.peac.org.ph

While waiting for the online voucher application portal to be accessible, may we request you to provide the following
needed requirements so that we can easily help you to apply when OVAP (online voucher application portal) is open.
Scan or take a picture and upload the following:
1. Recent 2x2 colored ID photo
2. The student should provide a personal email address. Please keep your password so that you can open your
account anytime.
3. Proof of financial means of BOTH parents or guardian/s, and the other person/s helping send the Voucher
Applicant to school, if any
4. One needs to digitize or scan all the required documents and submit one scanned file per document.
5. Please keep your scanned documents and submit them once the portal for application is open.
6. PLEASE ENSURE THAT ALL DOCUMENTS ARE CORRECT, READABLE, CLEAR, AND HAS BEEN ISSUED WITHIN THE
LAST 6 MONTHS.
Required documents should be for both parents either employed or unemployed.
