Dear Sir/Ma’am:

On April 15, 2021, Malamig Elementary School (MES) posted a request for quotation in PhilGEPS for the
supply and delivery of school supplies and equipment through Shopping (Sec. 52.1, IRR of R.A. No. 9184)
with Approved Budget for the Contract (ABC) of Seventy Thousand Eight Hundred Fifty Four Pesos.
Closing date was set on April 19, 2021 at 10:00 in the morning and quotations were received from
seven(7) bidders including your quotation.

In this regard, I am happy to notify you that the contract for the supply and delivery of school supplies
and equipment is hereby awarded to you as the bidder with Lowest Calculated and Responsive Bid for a
total contract price of Fifty Eight Thousand Six Hundred Twenty Pesos (P58,620.00). You are therefore
required to submit to us this notice with your Signed “Conforme” hereon as a requirement of the
Contract Agreement in accordance with the accounting and auditing process.

Failure to comply with the delivery and requirements within ten (10) days shall constitute a sufficient
ground for the cancellation of this award.
