I Pushpinder Kaur W/o Baljinder Singh R/o V.P.O. Burj Sidhwan, Tehsil Malout,
District Sri Muktsar Sahib, PUNJAB, INDIA do hereby solemnly affirm and
declare as under :-
1. That I am the permanent resident of Village Burj Sidhwan, Tehsil Malout,
District Sri Muktsar Sahib, Punjab, INDIA since my marriage.
2. That I wish to travel to USA to met my mother for the sake of their health.
3. That all the documents provided by me in my file are original, valid,
authentic and genuine and issued by the concerned authorities which can
be verified at any point of time.
4. That in case of any fake or false documentation in the file I shall fully
responsible for the same.
5. That I shall leave USA and come back to India prior to the expiry of my
Visa provided by the USA Government and shall not be burden on USA
Government.
6. I have sufficient funds with me in the shape of landed property, cash and
Gold etc.
7. I shall abide by all the COVID-19 restrictions imposed by the USA
Government.
8. That all above information provided by me is correct and genuine.
