ISBAR Method

1. Introduction
The nurse introduces herself to the patient and mentions the room where the nurse is on
duty
2. Situation
The nurse states the patient’s identity, room, room number, and the patient’s diagnosis
3. Background
The nurse mentioned supporting data:
 Patient problem intervention
 History of allergies, surgery, etc.
 Installation of tools and invasive procedures (catheters, infusions, etc.) as well as
administration of drugs, fluids
4. Assessment
 Describe the results of the patient’s assessment (blood pressure, nutritional status,
pain score, level of awareness, elimination, etc.)
 Describe supportive clinical information
5. Recommendation
 I asked the doctor to :
- Transferring the patient to the ICU?
- Come immediately to see the patient?
- Represent other doctors to come?
- Consultation with another doctor?
 What tests or therapy are needed :
- X-rays?
- Blood Gas Analysis?
- EKG examination?
- Giving Oxygenation?
 - A nebulizer?

 If there is a change in therapy then ask:

- How often are vital signs checked
- If there is no response to therapy, when should you call the doctor again?
 Confirm :
- I have understood the patient’s plan of action
- What should I do before the doctor gets here?
