1. I am an employee of the Department of Education Region 4A

CALABARZON who received the copies of the PIVOT 4A Learner's
Material Module in soft copies for printing, reproduction and distribution to
DepEd officials, personnel and learners only.

2. I hereby undertake not to divulge, publish, circulate or share in any manner,

the module, in part or in whole or a copy of which to any person or
institution without written consent to the Chief of the Curriculum &
Learning Management Division of DEPED 4A.

3. I understand that any violation of this undertaking shall be a ground for

DepEd Region 4-A CALABARZON to file civil, criminal or administrative
complaint against me before competent court, office or quasi-judicial body.
