AUTHORIZATION FOR ENROLLMENT OF VEHICLE TO THE

AUTOSWEEP RFID SUBSCRIPTION

(IF VEHICLE IS NOT UNDER THE SUBSCRIBER’S NAME)

This is to authorize (Mr./Ms./Company/Business) ____________________________________________ (the

“Subscriber”) to enroll the vehicle with details below to the Autosweep RFID Subscription (“RFID
Subscription”) under the Subscriber’s name with RFID Account No. _________________
(“Arrangement”).

Further thereto, we hereby confirm that:

• This Authorization includes the authority to sign, execute, and deliver, for and on behalf of the
Registered Owner, any and all application forms, authorities, letters, agreements, and other
documents and instruments as may be required for the enrollment of the vehicle into the RFID
Subscription and to pay, for and on behalf of the Registered Owner, any and all prepaid tolls or
unpaid tolls, fees, charges, and/or penalties as may be required or assessed on the above-
described vehicle, including those under a different RFID Account to which the vehicle was
previously enrolled.
• This Arrangement shall be on a continuing basis unless terminated in writing by the Registered
Owner, Subscriber, and/or by Citra Metro Manila Tollways Corporation.
• We are responsible in updating the vehicle details by submitting an updated Authorization with
the RFID Customer Service Center within fourteen (14) days from any change to avoid
termination of this Arrangement.
