Duty Hours 24+ Shift


ACGME 24 + Rule - Residents and Fellows are no longer limited to working just 4 hours after working a 24 hour
shift. According to the 2017 Rules, a Residents and Fellows may work as many hours as necessary after 24 hours
of continuous in-house duty seeing patients.
However, after 24 hours they cannot see patients. The next 4 hours are used for transition of care and then they
can continue work for exceptional circumstances.

Important Reminder
The additional 4 hours must be logged differently to avoid a duty hour violation. Acceptable options for the
additional four hours include: Day Off, Journal Club/Didactic, Post Call (in-house after night call/no new pts),
Research, Other. If you do not select one of these categories, it will be flagged as a duty hour violation. Please
reference the sample below for further guidance.
