Nursery - PSA Birth Cert Original only
KTN - PSA Birth Cert Original & Certificate of Completion (Nursery)

Grade 1 - ECCD Checklist, Card, Good Moral, Cert. of Completion & PSA Birth Cert Original
Gr. 2 – 6 - Card, Good Moral & PSA Original

Gr. 7 - Card, Good Moral, Cert. of Completion & PSA Birth Cert Original
Gr. 8 – 10 - Card, Good Moral & PSA Birth Cert Original (For new ESC student/s kindly please
submit your ESC Certificate to continue your contract).

Note: Form 137 & SF10 will be requested after they complied all the partial requirements requested by
our school. Parents/Guardian must be the bearer of the request form and the sf10/ form 137
documents.
